We are a passionate, Black-led, grassroots, volunteer organization working with families who are caring for a
loved one with a disability. We are committed to bettering outcomes for Black caregivers and those living with a disability. Sawubona provides a safe space for Black families to network, build connections, problem-solve, share resources, feel less isolated, and have others to talk to – who get it.
Mission Statement: The MISSION of Sawubona Africentric Circle of Support is to cultivate and improve the
self-efficacy of Black caregivers of persons with a disability and their families by providing necessary resources and support.
Vision Statement: Sawubona's VISION is for every Black caregiver of a family member with a disability to feel a sense of belonging.
Wajibu
The Service Navigator will:
● Conduct initial intake/assessments through various avenues such as face-to-face, telephone and email to determine the needs of families, interests, and next steps
● Provide individual case management support to help reduce barriers to services faced by families in the disability community, including attendance at appointments and meetings, advocating and intervening with service providers as needed
● Facilitate referrals to appropriate services, other professionals, agencies, services, or community resources appropriate to the individual/families' identified needs with a focus on culturally responsive supports
●Provide telephone support, crisis intervention and follow-up support to families as needed
● Work with families to identify support needs for their family members with a disability
● Support outreach efforts as needed to promote an awareness and understanding of the programs and
services provided by Sawubona ACS ·
● Ensure all attempts of contacts are documented while maintaining careful, accurate records and family files with all relevant information.
● Support the development of a toolkit of resources and advocacy supports
● Support with grant writing, proposal development; managing program budgets, reporting and accountability to funders
● Effectively managing communications with internal/external stakeholders and demonstrating discretion, integrity and professionalism regarding the handling of confidential matters
● Use strong computer and office productivity skills to undertake formatting and proofreading of documents, creation of correspondence, presentations and other documentation
● A satisfactory Vulnerable Sector Criminal Records check
● Perform other duties as required
Sifa na Ujuzi
● The ideal candidate will be someone with a strong interest in working with the Black community, working with families and/or supporting those with disabilities. It may be an individual who has completed postsecondary studies in psychology/sociology/social work, Social Service Worker, Developmental Support Worker or Child Development Worker with good knowledge of the human services system including housing, education, developmental services, health, etc.
● Understanding of institutional and other systemic barriers and how they impact Black families supporting individuals with disabilities
● Excellent written, verbal communication skills
● Professional demeanor with strong rapport, advocacy and relationship-building skills
● Strong quantitative and qualitative analysis skills
● Demonstrated ability to work independently and effectively allocate your time to complete important
productive tasks
● Strong organizational and priority settings skills
● Lived experience or demonstrated experience working with/within BIPOC/disability organizations/groups
● Proficient with Microsoft Office suite
● A satisfactory Vulnerable Sector Criminal Records check
● Access to a vehicle an asset
● Ability to speak French an asset
