Evaluation of our work needed
Hello everyone
This is a gentle reminder about the survey, original message noted below, that we are asking you to complete. So far we have only received a single response. We need some more data to go on and inform our future activities. We are asking for your feedback in regard to our online support group. Providing evaluation is an important part of us getting funding to continue to do this work so if you can please take 10 minutes to complete this survey, it would be greatly appreciated.
Deadline for completion is June 30. A gift card will be provided in appreciation for your time to complete the survey.

hi Everyone
I hope you are enjoying the start to June.
We are writing this to get your input and feedback in an effort to improve our work at Sawubona to meet your needs. As part of the funding we get from Ontario Trillium Foundation, we are provided with Evaluation support from the Student Commission.
We are seeking your feedback on one of our most important programs, the Virtual meetings that we facilitate 2x/mth. Between now and June 30, we are asking you to take a few minutes to answer a few questions through this survey link:

The first time you enter the site, you will need to Register
You will then get to a Consent page which is a mandatory part of any evaluation process to inform you about your rights when participation in research and evaluation
you will then get to a screen where you will be asked to complete a Demographic Profile which you will see on the main page called TOOLS, beside the MY PROFILE section.
Complete this Demographic profile.
once you complete this, LOG OUT then Log back in again. you should see another link for the Virtual meetings survey. It is s short series of questions asked and once you respond, it will ask you to SUBMIT your responses. This will be the end of the survey.
We will provide a gift card to each participant who completes the survey. Thanks for participating.
